'Conditions for Sanction' for Tournament Directors
In order for tennis facilities and tournament directors to have their tournaments sanctioned by the USTA Pacific Northwest (USTA/PNW), they are required to complete and agree to the terms outlined in the 'Conditions for Sanction' form. Tournaments that do not submit this form WILL NOT be sanctioned. Please click here to access the Conditions for Sanction form.
The Conditions for Sanction form must be returned to USTA/PNW Tournament Management Coordinator Becky Tallent at the USTA/PNW office via email at beckyt@pnw.usta.com or fax at (503) 520-0133. Forms may also be mailed to 4840 SW Western Ave, Suite 300; Beaverton, OR 97005.
